SOAP notes

What goes in a note after every service, and why the next therapist depends on it.

The four letters

S — SERVICE. The type and duration. “60 min Paloma with aromatherapy.”

O — OBSERVATION. What you found. Tightness, range of motion, flexibility, what she reported. More is better than less.

A — ACTION. What you recommended, products included — so nobody offers her the same thing twice.

P — PLAN. What you suggest next.

Completed after every service, without exception.

What goes in the Plan

Awaiting validation

What to write in the Plan line is something your trainer will go through with you before you write one on your own.

Where the note lives

Awaiting validation

Where notes are written and who can read them is part of the systems walkthrough in your first week.
